For purposes of this policy, "Portal" refers to the secure website used by clients to access Company services, "Website" refers to this informational website, and "Client" refers to an individual or entity that utilizes the services of Company.

Public Website Information

The Website is intended to provide general information about the services offered by SureCheckUSA. The Website does not collect or store personal information about visitors for service delivery purposes.

Information Provided Through the Portal

A Client may provide personally identifiable information ("PII") through the Portal if search result resolution services are requested. Depending on the service requested, that information may include:

  • First, middle, and last name
  • Address
  • Email address
  • Social Security Number
  • Date of birth
  • National Provider Identifier (NPI)

The Portal allows a subscribing Client to access Company services. Information provided by the Client may include name, address, date of birth, Social Security Number, and NPI. This information may also be provided for individuals or entities associated with the Client.

To use the Company's search service, a Client must provide a first and last name for an individual search or a company name for a business search.

If the Client requests that the Company perform search result resolutions, the Client may also need to provide date of birth, Social Security Number, and NPI, depending on the requirements of the applicable data source. A Client may instead choose to perform its own resolutions of search results and, in that case, may not need to provide additional information.

Portal Usage Information

The Company may collect operational and account activity information associated with Portal usage, including:

  • Login, logout, and other usage date and time stamps
  • Service requests
  • Additions and changes to profile information
  • Acceptance of portal website usage agreement

How Client Information Is Used

Information provided through the Portal may be used to:

  • Provide services requested by the Client
  • Notify the Client about changes to our services
  • Carry out obligations and enforce rights arising from contracts
  • Inform the Client of other services available from the Company

Sharing Information with Third Parties

The Company does not sell, rent, or lease customer lists to third parties. Information provided by a Client in the Portal may be shared with data source designated contacts or entered into data source-provided resolution web pages solely for the purpose of determining whether the subject of a search result matches a Client profile.

Security of Personal Information

The Company secures data from unauthorized access, use, or disclosure through encrypted communications, database encryption, and other administrative, technical, and operational safeguards.

Email Communications

From time to time, the Company may contact Clients by email regarding announcements, service-related communications, promotional offers, or other business communications.

Changes to This Privacy Policy

The Company reserves the right to update this Privacy Policy. Clients will be notified of significant changes through email or through a prominent notice on the site, as appropriate.
